Loan Application Checklist

Requested documentation can be securely uploaded after completing your online application.

 

You will need the following information to complete the application:
Name, social, date-of-birth, residence and work history for all applicants for two years.

Property Information:
Property address.  Please provide a copy of the purchase agreement if you are purchasing a home and have an accepted offer to purchase a specific property.
If you've recently sold a home, provide a copy of your Final HUD-1 Settlement Statement or Final Closing Disclosure (CD).

Assets and Income For All Applicants On The Loan:

Official statements for each bank, mutual fund, and/or investment account for the last two months.  Statements must be consecutive and include all pages.  For example, if the statement shows "Page 1 of 5," all five (5) pages must be provided.  
 

 

Income for wage-earners:
 

Paystubs for the most recent month covering one month of earnings.

W-2s for the most recent two years.
  If you own more than 25% of a corporation or partnership:
 

Federal corporate or partnership tax returns including all schedules filed for two years.

Year-To-Date Profit and Loss and Balance Sheet required if applying for FHA loan.
  If you are a Sole Proprietor filing a Schedule C:
 

Personal federal tax returns for the last two years with all schedules filed.

Year-To-Date Profit and Loss and Balance Sheet if applying for FHA loan.
  If you own rental property:
 

Personal federal tax returns for the last two years including all schedules filed and current rental agreements.
  If you are retired:
 

Pension Award Letter.

1099s for two years.
  Additional sources of income:
 

Provide documentation supporting receipt and continuance of this income.
 
 

Debts

Names, addresses, account numbers, balances and monthly payments on all current loans. (WE WILL PULL YOUR CREDIT SO THIS ONLY APPLIES IF YOU HAVE RECENTLY OPENED NEW DEBT THAT DOES NOT APPEAR ON YOUR CREDIT OR YOU HAVE MONTHLY OBLIGATIONS NOT REPORTING ON YOUR CREDIT.  DO NOT INCLUDE UTILITY BILLS.) 
Explanation of credit report anomalies, including:
 

Late payments, credit inquiries in the last 90 days, charge-offs, collections, judgments and/or liens.

Bankruptcy or foreclosure filed within last seven years (provide a copy of your bankruptcy papers).  Short sales completed in the last four years (provide Settlement Statement).

VA Loans

Copy of DD Form 214, Report of Separation.
CHA Mortgage Company will request your Certificate of Eligibility from the VA.

Miscellaneous

HOA, property tax and hazard insurance statements for properties owned if these are not included in the monthly mortgage payment.
Legible copy of Photo ID.
If you are not a US Citizen, please provide a copy of the front and back of your Resident Alien Card or Visa.
